You are no longer restricted when entering codes or administrative actions (except in extreme cases such as murder, first degree arson, etc.). Please be sure to let Arlene Fowler know if you receive an error message when entering your discipline transactions.

 You can complete the form once it is displayed on your screen. Once completed, you may either print or save the form.

If you would like to save the completed form:

• Name this file and save it to a directory on your hard drive.
